MEPS checklist — what to bring.

• Photo ID (driver's license or state ID).
• Social Security card or proof of SSN.
• Birth certificate (original or certified copy).
• Medical records: surgeries, prescriptions, mental-health treatment, glasses prescription.
• High-school diploma, GED certificate, or transcripts (college transcripts if applicable).
• Marriage certificate, divorce decrees, and dependents' birth certificates if you have any.
• Disclosure of any prior arrests, charges, or convictions — including those expunged.
• Sleep at least 7 hours; eat a normal breakfast; no caffeine binge.
• Wear comfortable, conservative clothing; bring a backup outfit.
• Bring contact lenses if you wear them; you may need to remove them for the eye exam.
